NHS England/Improvement (NHSE/I) versions of the updated pan-London Medication Authorisation and Administration Record (MAAR) for use in Palliative and EoLC for subcutaneous and intramuscular medication in the community setting are now used across North East London (NEL) by acute hospitals,GPs, hospices, community services and out of hours services. All Primary Care practices are required to import the attached EoLC MAAR chart templates into their relevant GP system.
In case of issues accessing the template Palliative & EoLC MAAR Chart via your clinical system at any time, copies of the chart can be used as a back-up (please see links to Word and PDF versions below.)
Do not make any unnecessary amendments that might change the over-all format / look of the chart. Once embedded please ensure that any changes you have subsequently made (if any) do not change the over-all alignment. This is particularly important for SystmOne and Vision when mail-merging from the word document. For example: in the EMIS formatted version the end-user is referred from the allergy section on page one to the end of the document where an auto-populating section has been added to accommodate the multiple entries whilst retaining the over-all document alignment.
Please check that when printing the chart the patient demographics appears at the top of every page and that where multiple lines of free-text has been added to a section that all lines remain readable. The prescriber is therefore encouraged to limit detail to essential detail only. Patient demographics must appear at the top of all pages – failure to do this will result in the chart being REJECTED by community end user services and could compromise and delay care.
