The following guidelines have been approved by the Barking and Dagenham, Havering and Redbridge Area Prescribing sub-Committees (APC). They have been developed in association with the relevant clinicians in both primary and secondary care, where applicable.

    Please see below for various information on Freestyle Libre

    The forms below are for REFERENCE ONLY and not for GP practices to fill out and complete. When a specialist sends a request to prescribe Freestyle Libre, you should receive:

    and ONE of the following prescribing request forms clearly indicating whether the patient meets the criteria (Indications 1,2 or 3) to prescribe for up to 3 months before review in the clinic:

    After the patient has been prescribed Freestyle Libre in primary care, the patient will be reviewed in the clinic. If outcomes have been achieved, a request for long-term prescribing will be sent to the GP (see form below):
