The Community Falls service is now accepting referrals from GPs, other healthcare services, residential homes and voluntary sector services in Redbridge. This means that the service is available to all residents across BHR who meet the service criteria.
The BHR Community falls service is aimed at preventing and reducing the incidence of falls in the community whilst minimising the impact these may have on service users and their carers. The service is multi-disciplinary led and includes input from a consultant geriatrician, a specialist falls nurse, physiotherapists, occupational therapists and rehabilitation assistants. The service provides a range of clinical assessments, diagnoses, treatment, rehabilitation and health promotion, delivered via one to one, group, and education sessions.
The Community Falls service is for people aged 18 and over who are registered with a GP in Barking & Dagenham, Havering, or Redbridge, and who meet one or more of the following criteria:
