The Chairs’ Pilot in Primary Care – Opportunity for PCNs

The BHR CCG Chairs have developed an exciting opportunity for Primary Care Networks across Barking & Dagenham, Havering and Redbridge to propose, develop and pilot new and innovative ways of working to improve the delivery and quality of primary care services to local people - the Pilot in Primary Care (PiP). The purpose being to give PCNs, on a small scale, an opportunity to test ideas and new ways of working (via a quality improvement approach) to key areas of focus. If these pilots are successful, they can then be rolled out across BHR.
The process to apply for funding will be simple, and framed around the key areas set out below. Interested PCNs should submit proposals to Natalie Keefe (natalie.keefe@nhs.net) by 30 October 2019.
Principles
- PCNs to frame proposals around the key areas identified below – to pilot different ways of working to achieve a measurable improvement to patient care
- Proposals should include intentions around how you would share your innovation if successful, with other PCNs across BHR at a PLE/PTI
- At least three PCNs per borough will be awarded funds to take forward their proposal (excluding Shared Care)
- Funding for successful bids will be awarded on a 65% upfront, 35% on delivery of outcomes
- The pilot will run from November 2019 to March 2020; with outcomes to be presented at a second ‘Chairs’ Den’
- The process to apply is simple and non-bureaucratic
Process to Apply
- With the exception of Shared Care, all funds available will vary on the size of the PCN to a maximum of £30k per proposal
- Shared Care will have a maximum of £5k available for PCNs to develop a specification for a LIS to implement in 2020/21 – only one PCN to be awarded this per borough
- Interested PCNs need to submit a one page proposal on any of the key areas below. PCNs can submit more than one proposal
- A lead from the PCN will provide a 5 minute pitch regarding their proposal which will include; outline of the proposal, investment required, expected key outcomes, timeline for delivery
- The pitch will be heard by the CCG Chairs, an independent clinician and Lay Members at a first Chairs’ Den during 7 November 2019
Areas of Focus for PCNs
- Prevalence: Reduce prevalence gap by 10% in three of following LTC; Diabetes, Hypertension, COPD, osteoporosis
- Urgent care: Solutions as to how practices can manage urgent primary care requests in different ways within a PCN-setting (in hours/extended hours)
- Dementia: Reduce prevalence gap by 10%, increase frailty index recording, increase number of completed care plans by 10% (B&D and Havering only)
- Technology & Innovation: Increase NHS App & GP Online registrations (by 10%) and number of e-consultations (all practices within a PCN providing the service and proposing a % increase utilisation target)
- Prevention & screening: Improve immunisation rates (particularly for MMR) and cervical screening – to achieve national targets
- Mental Health: Increasing the number of IAPT referrals (pitch your proposed PCN target at the Den!)
- Shared care: Develop a proposal for how Shared Care could be provided in a primary care environment (we are looking for one PCN in each of the CCG areas to participate in the PIP (supported by Julia Cory, interim Deputy Director, Primary Care Transformation)
