As you know Barking and Dagenham, Havering and Redbridge, (BHR CCGs) are putting in place more efficient and effective IT systems for GP practices to manage or support patient health checks, children’s immunisation services, retinopathy screening and care plans.
Because of these changes your practice needs to take action by the end of this month to ensure you still receive payment for the health checks.
The new system, Discovery, will automatically pull off the health check data from your practice and process it. The system will also automatically ensure you get paid for the health checks. The existing system for health checks, Health Analytics, will not be used from 30 September 2018.
The intension is for the Discovery to share Rio data, so the traditional Health Analytics care plans will be replaced with Rio care plans. BHR CCGs have also invested in end of life software and will be introducing Coordinate My Care to manage end of life care plans.
The systems are all cloud based and will only require you to login, in the case of Coordinate My Care, or enable connection in the case of Discovery by the 30 September.
Don’t miss out on your payments
In order for this transition to happen smoothly and your practice to continue to receive the health check payments from 1 October, you must follow the actions below before 30 September:
If you previously used Health Analytics to run queries on patient numbers from 1 October please use GP Clinical Systems to do this instead.
