Dear colleagues,
In response to local health needs and national guidance, BHR CCGs and our local providers have reviewed the need to introduce an acute Diabetic Foot Care Pathway.
BHRUT and NELFT colleagues have collaborated to develop a Diabetic Foot Multi-Disciplinary Team, with dedicated clinics starting this week (from 11 November)
These will be available at both Queen’s and King George Hospital sites. These clinical sessions are devoted to patients experiencing limb-threatening and life-threatening diabetic foot problems. There will be open referral access to the MDT via a dedicated e-mail address (bhrut.sm-diabeticfootmdt@nhs.net).
GPs, Podiatrists and Emergency Departments must now use this pathway to reduce the emergency admissions rate for our diabetic patients. Within this service, there will be access to dedicated Podiatrists, Vascular surgeons and Diabetologists, and orthopaedic input available as required.
You can download the Diabetic Foot MDT referral pathway from the Diabetes foot clinic page on your GP intranet
Also available to download is the referral form, indicating referral criteria and clinical presentations suitable for these clinics. The dedicated email address for Diabetic Foot Multi-Disciplinary Team referrals is: bhrut.sm-diabeticfootmdt@nhs.net
You can find the access referral guidance and forms to NELFT Community Podiatry Services at https://www.nelft.nhs.uk/home
This service will greatly benefit our local population of people living with diabetes experiencing serious diabetic foot problems. We expect this new service will:
This is an exciting development of collaborative working between the CCGs, BHRUT and NELFT.
Please note: this is not a replacement to existing NELFT Community Podiatry Services based within Barking & Dagenham, Havering and Redbridge – and referrals should continue in the usual way.
Thank you for your support
