As you know, Barking and Dagenham, Havering and Redbridge clinical commissioning groups (BHR CCGs) are looking to roll out a new enhanced healthcare service for residents in local nursing homes.
This service, which we hope will begin in autumn 2018, will be based on the findings from the CCGs’ two-year Health 1000 pilot and will see dedicated GPs working more closely with the residents and staff in the homes to help more local people be cared for in their nursing home, rather than hospital.
The Health 1000 pilot trialled new ways of providing coordinated health and social care for local people who have five or more long term conditions and for four local nursing homes. The nursing homes taking part in the pilot saw significant benefits to patients, including a reduction in A&E attendances and hospital admissions amongst their residents.
BHR CCGs are now working with clinicians and using the evidence and feedback from nursing homes to shape the new service, which will deliver improved patient care to nursing home across the three boroughs, and to run a procurement process. We intend for the service to be rolled out in a phased way over the next few years to all homes
Our priority is to ensure that patients continue to receive high quality, uninterrupted care. We are writing to all patients to inform them of the decision to commission the new service and to reassure them that they will remain registered with Health 1000 until the new service begins.
The patients with five or more long-term conditions who are not a resident of a nursing home but who are currently registered with Health 1000 will continue to be supported and will be offered the option to register with the new provider of the nursing home service when they are chosen later this year. We are writing to all patients to inform them of the decision to commission the new service and to reassure them that they will remain registered with Health 1000 until the new service begins.
Where possible, this process will be managed with the support of the patient’s existing Health 1000 key worker or their GP.
