Local community voluntary organisations are playing a huge role in supporting the health and wellbeing of our local frail population’s health and well-being. Their collaboration is crucial to the transformation of care, and they are working closely with all partners in our transformation board.
Both the British Red Cross and Age UK were instrumental in the success of a recent 10 day trial of the Home-Is-Best model, from Queens Hospital that saw nineteen hospital admissions avoided. Their support helped individuals return home quicker and also supported them once home, through their visits, that offered advice and guidance on how to access appropriate community based services that can reduce social isolation and prevent re-admission.
Age UK continues “Strength & Balance” exercises groups in Redbridge and now in Barking and Dagenham and in Havering as part of a pilot falls prevention scheme aimed to help older people avoid falls.
Age UK is also trialling a Care Navigator pilot to support the completion of “Coordinate My Care” plans for palliative patients in Redbridge, due to start in autumn 2019.
