Barking and Dagenham has been ranked 9th best in England in a league table rating performance on patient flow between the boundaries of health and social care. Redbridge and Havering also performed well, ranking 1st and 11th respectively in the list of 150 authorities.
This achievement is a direct result of the partnership work between our local authorities, the CCGs, NELFT and BHRUT to help patients to return home safely and quickly with the right support.
The ‘NHS social care dashboard’ provides rankings on six key performance indicators, including emergency admissions levels for over 65s (April 2017- March 2018), the percentage who receive rehabilitation (2016-17) and delayed days in hospital (April 2017-March 2018), which is given the highest weight in calculations.
This is one of the workstreams of the urgent and emergency care transformation programme, led by the BHR A&E Delivery Board. Together, we’ve focused on improving patient care and safety by bringing health and social care professionals together to plan individual patient’s discharge from hospital. This ensures local people have the right support in place and reduces delays to discharges for those needing social care or rehabilitation services.
