BHR HOS-AR Services
Home oxygen should only be ordered by staff trained in oxygen prescription and use. Across Barking and Dagenham, Havering and Redbridge, home oxygen in the community should be ordered wherever possible via the Barking and Dagenham, Havering or Redbridge Home Oxygen Assessment and Review Service (HOS-AR)
HOOF, IHORM and HOCF
The Home Oxygen Order Form (HOOF) A and B has been updated nationally. There is now a requirement to ensure an Initial Home Oxygen Mitigation (IHORM) form is completed as well as a Home Oxygen Consent Form (HOCF). Both these forms are available through the electronic portal and once completed should be printed and retained in the patient’s notes (see below). If a High Risk or 3 or more moderate risks are identified on the IHORM form it is highly recommended that oxygen is not requested without referral to your local HOS-AR service. The HOOF A and B now contain an additional declaration to show the HOCF and IHORM have been completed.
Electronic Portal for Ordering Home Oxygen via HOOF A and B

London Clinical Oxygen Network (LCON)
The London Clinical Oxygen Network has produced guidance on Cluster Headaches for GPs, neurologists and patients. Across BHR CCGs, GPs are not expected to prescribe oxygen for cluster headaches and any such requests should be directed to the local HOS-AR services. However, the informationwithin the guidance may be useful for GPs to note, especially the diagnosis support between cluster headache, tension-type headache and migraine. Also, the patient information may be useful to support any queries from patients on oxygen for cluster headache.
