Alert the CCG

You can raise concerns about a number of issues, such as the safety or quality of service, or about specific instances where a patient has had a poor experience of care, or has not received care. You can access the alert form below. 

The CCG team will compile a monthly report of alerts received and pass these through to the central BHR CCGs’ Nurse Director who is responsible for quality, safety and safeguarding. The alerts will be reviewed for necessary action and any themes that require further investigation will be identified.

Please note that the GP Alert system is not intended to replace the complaints mechanism, nor is it for reporting serious clinical concerns should not be used for urgent issues requiring immediate action. 

An individual GP response will be provided if requested when completing the GP alert form, as well as an update provided through the regular GP locality forums about any operational changes made by providers as a result of the GP alerts being raised.  

We will continue to keep you updated and welcome any feedback that you might have on how to improve the GP Alerts system.

To investigate this issue, we may need to share patient identifiable data (PID) with the provider involved in this issue. We will request this directly from you/ the practice – please do not enter any PID into the form itself. Please ensure that you have explained this to the patient and that you have documented their consent (verbal or written) to share PID information relating to this alert with NHS Barking and Dagenham, Havering and Redbridge CCGs for review and onward transmission to the patient’s healthcare provider.
